Criminal History Disclosure Form - Form

The volunteer program provides opportunities for community members to actively support the City with various projects and programs, which serve the citizens of Woodburn. The significant contribution of time, energy, special skills and expertise are respectfully appreciated and your help allows the City to offer a higher level of service to the citizens of our community than we would otherwise be able to provide. Once an individual submits their application and Criminal History Disclosure Form, there is a review process, assessment of best "fit" for volunteering and coordinating for an orientation, training and placement at a site. Background checks will be completed on individuals 18 an older.
